Guidelines for Dream Granting

1.  To be eligible for a Dream Grant, an applicant must meet the following
     criteria:

     a.  Be at least 19 years of age but not more than 60 years of age.

     b.  Have resided in an eligible municipality for at least one (1) year.

     c.  Have a life threatening illness as determined by a doctor.

     d.  Have not previously received a wish/grant from any other granting
          organization.

 

2.  An applicant must complete the following forms:  Application, Liability
     Release and Publicity and Disclosure Authorization, Medical
     Authorization, and such other forms as may be required by the
     Foundation.

 

3.  The Foundation Board of Directors shall select all Dream Grant
     recipients.  The Board will make a determination within thirty (30) days of
     receipt of a complete application.

 

4.  No grant shall exceed $7,500.00 in value.  There will be no cash grants.
     Grants may not be used for medical purposes.

 

5.  To be an eligible municipality, a municipality must meet the following
     criteria:

     a.  Be an incorporated municipality or township in the State of Wisconsin
          with a population of 3,500 or less.  (An eligible municipality whose
          population increases to more than 3,500 after being deemed eligible
          will remain an eligible municipality.)

     b.  Have designated contact person who resides within the municipality.

     c.  If located within Ozaukee County or Sheboygan County, make an
          initial contribution to the Foundation of at least $1,000.00.

     d.  If located outside of Ozaukee County or Sheboygan County, make an
          initial contribution to the Foundation of at least $5,000.00, and conduct
          at least one annual fundraising activity from which the proceeds are
          contributed to the Foundation.
